Notice of Physician Departure

This letter is to notify you that Aaron Farberg, M.D. is leaving Derm Texas. Dr. Farberg and Derm Texas LLC are committed to ensuring that all patients of Dr. Farberg have access to appropriate medical care during this transition and that you receive adequate notice of your rights to request information and exercise unfettered patient choice in choosing your physician.
Dr. Farberg will continue to be available to provide medical care at Derm Texas until at least January 31, 2023. After that time, you may continue to receive medical care at Derm Texas from Kelly Warren, M.D.
Following Dr. Farberg’s departure, he will continue to be available to see patients at Bare Dermatology, (opening at 2110 Research Row, Floor 3; also Rockwall and Forney) and he may be contacted at (972) 213-6210 or at doctor.farberg@gmail.com or at www.barederm.com
Kelly Warren, M.D. at Derm Texas LLC is the current custodian of your medical records. However, you are entitled to request copies of those records or to request that they be transferred to a physician of your choosing (including Dr. Farberg). Please direct any requests regarding custodianship of your medical records to Amanda Harvey at aharvey@dermtexas.health or call her office at 214-847-6426.
Dr. Farberg’s and Dr. Warren’s first priority is ensuring that you receive quality medical care. If you have any questions or concerns, or if you want to request medical services or special accommodation relating to this transition, please do not hesitate to contact us.
